Add-on Jersey Personalization (Name and Number)

This is for personalization only.  You must also order a jersey.

STEP 1:
Put your Name and Number in the “Special Instructions for Seller” box in your shopping cart before checking out.

STEP 2:
If you order more than one jersey to be personalized, please also add in the notes which personalization goes with each jersey/size (for anyone ordering multiple jerseys, we will automatically email you to reconfirm each personalized item).

STEP 3:  Confused on where to put the note?  Just order the jersey and the personalization and we will email to get the name and number.
